New Patient Referral Process
Physical Therapy services provided by Play at Home Physical Therapy require a prescription from a physician. If your insurance is listed in the accepted insurance plans below, Play at Home PT will verify that your child's insurance coverage for physical therapy services. The following documentation will be required in order to verify your insurance:
- Physican's Prescription
- Referral from Physician
- Insurance card
- Subscriber's Driver's License
Accepted Insurance Plans:
- AmeriHealth
- Highmark Blue Shield
- Independence Blue Cross
- Keystone Health Plan East (HMO)
Insurance not listed? No problem!
If Play at Home PT does not accept your insurance, we can still provide physical therapy services to your child.
Private Pay Option
You will pay out of pocket for your child's physical therapy services before any appointment begins. Our prices are fair and reasonable and can be provided upon request.Out-of-Network Option
In this situation, Play at Home would be considered an out-of-network provider. You accept financial responsibility, pay up front, and would have the option to seek reimbursement from your insurance company after the treatment.Prior to your appointment, Play at Home will verify your out-of-network benefits. The level of reimbursement varies by health insurance plan. Play at Home will provide you with a complete, detailed receipt for services that you can submit to your insurance carrier for reimbursement. Play at Home will provide assistance as necessary to submit your claim.
