We have made the decision to transition our practice to an out-of-network provider for pediatric physical therapy (PT) and occupational therapy (OT). This decision was not made lightly, but it is necessary to ensure we can continue delivering the highest quality of specialized care for your child and family.

The constantly decreasing reimbursement rates from insurance companies have created significant challenges that often compromise the standard of care we are committed to providing. By operating outside of insurance networks, we can protect the quality, frequency, and duration of the therapy your child receives.

Complete Intake Forms

Fill out our comprehensive new patient intake forms online. This helps us understand your child's needs before the first visit.

Obtain a Script

Request a current PT and/or OT prescription (script) from your child’s pediatrician.

Schedule

Once we have received your completed forms and the physician's script, your forms will be reviewed. You will then be connected with your dedicated therapist via email to discuss goals and get scheduled for your first appointment at the location of your choice (home or clinic).
